<p>Job Overview:</p> <p>The stroke coordinator is an expert clinician who provides leadership for the nursing care and management of patients experiencing stroke. The stroke coordinator works with the Stroke Program Manager, Medical Director and other members of the stroke team to ensure all patients of TriHealth receive high quality evidence-based care by providing expert nursing consultation and practice oversight. The coordinator, in concert with the stroke program team, develops and ensures relevant stroke initial and continuing education is disseminated to all team members who care for stroke patients. The coordinator leads performance improvement projects developed based on relevant and monitored stroke metrics. The coordinator works with an interdisciplinary team to develop, implement, and evaluate standards of care, protocols and order sets to improve patient care, practice and/or systems. Collaborate with clinical leaders to ensure a well-coordinated, evidenced-based, multidisciplinary program of care and education is provided to all patients.</p> <p>Location:</p> <ul> <li>Good Samaritan Hospital </li></ul> <p>Work Hours:</p> <ul> <li>Full-time position: 80 hours bi-weekly </li><li>Day shift </li></ul> <p>Job Requirements:</p> <p>Bachelor's Degree in Nursing (Required)</p> <p>5 - 7 years experience Clinical Nursing (Required)</p> <p>Microsoft Office (PowerPoint, Excel, etc.)</p> <p>Registered Nurse - COMPACT (Multi-State) Required</p> <p>RN Specialty Certification ABNN offers CNRN, SCRN ANVC offers ASC-BC, NVRN-BC ANCC/ANA offers CV-BC, or relevant certification within 3-years from hire. Required</p> <p>Job Responsibilities:</p> <p>Ensure maintenance of stroke certification for assigned location. Support all entities of the Joint Commission Certification process to include biannual survey, annual intracycle calls, and quarterly data entry.</p> <p>Work with prehosptial partners Collaborate, create and present (in-person, livestream, written) current evidence-based education to prehospital providers, TriHealth team members, community members, and others within the realm of stroke systems of care.</p> <p>Proficiency in AHA/ASA Get with the Guidelines stroke website and registry and coordinate with data analysts and abstractors to meet agreed upon reporting requirements.</p> <p>Collaborate with service line, unit, and medical leadership at the hospital site in the development, modification, and adherence to stroke clinical pathways, process improvement, program standards, clinical outcomes, external regulatory requirements and best practices utilizing relevant/current data, chart review, and responding to stroke alerts (ED/inpatient) to decrease stroke length of stay, morbidity and mortality.</p> <p>Collaborate with the stroke Medical Director and program manager to facilitate the achievement and maintenance of required clinical metrics for the stroke program.</p> <p>Responsible for all entities of monthly stroke committee meetings, process improvement subcommittees, stroke case reviews, etc. to include creating agendas, PowerPoints, and meeting minutes. Ensure appropriate participants attend meetings as required by TJC.</p> <p>Excellent organizational skills including ability to work independently and set priorities. Maintains professional knowledge and skills through nursing practice, in-service, and continuing/formal education. Obtain contact hours to maintain licensure and as a member of the core stroke team.
