NFP offers the opportunity to provide care to underserved and marginalized populations in a Community Health setting, contributing to personal and professional satisfaction.</p> <p>Schedule and Hours:</p> <ul> <li>Full-Time: 4-5 days a week </li><li>Part-Time: 2-3 days a week </li></ul> <p>Compensation: $100,000 to $121,000 based on Experience, and FTE.</p> <p>Position Overview:</p> <p>In collaboration with Physicians, the Advanced Practice Provider (APP) is an Advanced Practice/Certified Nurse Practitioner (APRN, CNP) or Physician Assistant (PA) that provides direct patient care, including preventative, primary care, or chronic care services.</p> <p>Essential Duties:</p> <ul> <li>Clinical Practice: Perform patient assessment, using clinical judgement for the assessment, diagnosis, or treatment of patients, including prescribing medication or other treatments. </li><li>Patient Education: Provide appropriate education to patients or family members to assist in the ongoing care and treatment of the patient. Update and modify plan of care as conditions evolve. Collaborate with community agencies to plan comprehensive care for patients served. </li><li>Compliance: Maintain up to date and accurate patient records. Participate in annual chart review, biannual prescriptive review, regular peer review, or other performance improvement activities as assigned. </li><li>Other Duties as Assigned </li></ul>
