The ideal candidate will be passionate about community health and improving access to vision care. You will provide full-scope optometric services to patients of all ages in an outpatient clinic setting, with a focus on patient education, prevention, and chronic disease management. To learn more about First Choice Health Centers, visit us at firstchc.org.</p> <p>Minimum Knowledge, Skills & Abilities Required:</p> <ul> <li>Doctor of Optometry Degree </li><li>Completion of Optometry program accredited by the Accreditation Council on Optometric Education of the American Optometric Association </li><li>Valid and unrestricted Connecticut License </li><li>Federal and State controlled substance licenses/state narcotic registration </li><li>Ability to work as part of multi-disciplinary team </li><li>Desire to serve a diverse population in a community health setting </li></ul> <p>Experience and Training:</p> <ul> <li>Minimum of 1-2 years of clinical experience preferred, but new graduates are welcome to apply </li></ul> <p>Standard Job Duties:</p> <ul> <li>Perform comprehensive eye exams, including refractions, ocular health evaluations, and contact lens fittings </li><li>Diagnose, treat, and manage ocular diseases and conditions </li><li>Work collaboratively with primary care providers and specialists to manage systemic diseases with ocular manifestations (e.g., diabetes, hypertension) </li><li>Provide patient education and counseling on eye health, hygiene, and preventive care </li><li>Document all patient encounters in the electronic health record (EHR) in a timely and accurate manner </li><li>Participate in clinic quality improvement initiatives and outreach efforts as needed </li><li>Refer patients to ophthalmologists or other specialists when advanced care is required </li></ul> <p>COVID-19 considerations: Employees of First Choice Health Centers must be vaccinated against COVID-19.
